I didn't even know this was a feature until I saw a sim have the orgasm moodlet while watching a stream, I do have the patreon version of ww and I have the visual orgasm effect on but I have yet to have a sim have an orgasm. Is it another mod that I have to have on top of ww?

You don't need another mod. I believe it's a function of the sexpertise skill - ie high partner sexpertise means faster climaxes. I think it's also affected by the trait that makes a sim a people pleaser or a selfish lover - I forget what these are called.
