There's utilities that allow you to draw onto a 3d mesh and it handles the UV translation onto the 2d texture for you. Blender for instance can do it. Blender is obviously not a paint program, so the utilities for drawing may be limited. I'm not aware of any utility that would be perfect for this. 1

To actually do what you ask for, you would have to build a real-time two-way bridge between a 3d software like Substance Painter or Blender and Skyrim. And unless you game on a workstation level machine with high resolution touch screen, or with with graphical tablet at hand and plugged in (assuming Skyrim even will work with those inputs out of the box), you'd find actually writing something a challenge. SlaveTats with their packs are probably the best thing you get for that functionality. You can use MCM, or apply those via scripts. You will be limited to premade textures however. I'm not aware of any utility that would be perfect for this. Texture paint feature in blender does come with brushes, masks and other necceceties out of the box. Blender is awesome, its just... a bit of a swiss amy knife, strapped onto chainsaw-equipped robot vacuum cleaner. The sheer amount of functions makes it daunting. There are plugins as well if you want a steam-powered sewing machine attached to that chainsaw, ot jet propulsion. Exceedingly powerfull freeware.
