I'd start by removing that duplicate mod file.

Removing the duplicate should be done but that likely isn't causing your issue.

 

The BE you sent earlier isn't even the BE, it's a PDF which breaks the functions and makes it impossible to see your problem. We need to see the html version. LL doesn't allow html uploads but if you just change the extension to something like txt, whoever downloads it can change it back and view it as intended.
