Sexbound Defeat not triggering

Yes, yes. I know, second topic I'm making in a short timespan but there's (another) issue I'm having.

 

It's about Sexbound Defeat. Whenever I get killed, it's really flakey whether or not I get into "defeat mode" or actually die. Sometimes it works. Sometimes it doesn't. Even against the very same enemies. And I'm not entirely sure what determines it. I know Defeat is a bit old and possibly not entirely up to date. Maybe that's a factor? If anyone's got experiences and insights to share I'm all ears (or eyes as the case may be).

Well, defeat is pretty much THE broken mod, every person has weird interactions with it, I'll suggest waiting till the mod is fully rewritten at some point

...I actually figured out why it happens (or doesn't happen) and now I feel stupid because it's right in the FAQ of the mod. I also forgot the very basics of the sexbound mod as well (that is, needing to be able to place a node).
