It appears to only occur with giants summoned by Hentai Creature spells.

For the moment I haven't find a Fix with Esl or Script, but it seems to be something with some script with Hentai Creatures or a Persistent flag, but if you want a fix for the moment, you need to have an "Animation Scene" with any vanilla Giantess before using the giantess from Hentai creatures, that at least worked for me, this maybe can fix the bug for the rest of a play thought, or only until you exit the game.

edit: (and that's without MNC, which I didn't get to work, but with fnis creature pack, slal and a bunch of anim packs)

Probably my mod should be replacing any skeleton mod you have like MNC, or you need cbpc and FNIS

I have cbpc and Fnis, I use MO2 so I just put your mod below the MNC, I don't even know what else to do

I edited the Skeleton of XPMSE, my mod should overwrite it, and MNC esp should be loaded after my mod

Quick question, perhaps I missed it in the description somewhere, but does this replace ALL giants with a futa version, or SOME giants with a futa version? I prefer some, as I'd like to have some diversity in creatures.

It replaces all giants with futanari versions and add non-futanari giantess, this edits the race itself so maybe if you want more diversity, make their own race would be necessary

Is there a version that only changes the Giantesses to Futa?  I want to keep the male ones too.
