Need menu restore item for USO [Dire situation!?]​

I don't know how to code at all and this mod contains everything that I need and can't find in other mods
But the menu is gone
I've tried everything besides a restore item, which I lack all the knowledge of creating
But surely someone could use an existing one as a template and just swap ids 

Id be SO appreciative
This mod is so essential to me

I've already tried uninstalling, saving, reinstalling
running menu cleaner and savings, and exiting
and to no avail
