weird "reacting to nudity" when there is none

getting this in 163c at times, but there is no nudity around.

this kind of happen when sim after using toilet, but not all times. like use/replace tampon/pad, maybe using toilet or bathing, their outfit is not selected (as with green background) in the ui outfit selection, but it does wearing clothes at visual.

i have to explicitly change it's outfit even it's the same one its already putting on, just to ensure the green mark is shown in the outfit selection.

i tested the game without mods, run 3 tests to use toilet, sim will have outfit selected as expected.

test with only ww mod, only the 2 essential files and empty setting folder, run 2 test using toilet, outfit is NOT selected.

test with 162g, same result, outfit NOT selected.

looks like latest game change something need ww to tackle with.

Hey im having a miscarriage problem, it doesnt work. the switch is on and they do drugs but nothing happens, its been like this for some time now. whats the problem?
