Well with a 3d modeled dick you can pose penis body and import them as bottom for sims 4  so it’s possible to do.

 Yes it’s possible, a lot of sims 4 downloads are on pages that nobody knows they existed, or could   be created   For example untraditional nerd, did one similar  it’s called detailed bottom.

The most realistic penises we've gotten are simdulgence, I really doubt these are real : P 

Might be possible in theory but in practice is a lot different.

Dunno if it's because you want really big penises or it's realistic look you're going for but personally think Simdulgence cocks look the most realistic in my opinion, that said I think Noir huge

Simdulgence RealgenPenisPlus xxl are probably only really big animation supported penises out there currently. Personally I prefer around XXL realgen or RealgenSimple XL if you have access to patreon exclusive over noirs much as I like the looks of noir's huge I feel XXL and XL from Realgen has less clipping during animations. Even if there still some it doesn't feel nearly as often to me. But probably a preference thing, if clipping doesn't bother you Noir's huge is likely biggest you can get with animation support.
