Hm. I browsed through my games library, but only found of a few.

 

Sims 3 with KinkyWorld. You can play play a gay sim or several - teenager and above. KW also allows you to set gender preference (percentages) for your town's population, yo you could play an entirely gay town. KW adds sex interactions, skills for sex and stripping, prostitution (a kind of mini career with whore / callgirl / escort), and optionally a playable High School for teen sims, exhibitionists, rapists and zoophile content if you have pets installed, plus many other features. You can find links and the free version here on Loverslab in the Sims sub forums.

 

Java Sim Brothel - JaSBro is a complex, turn-based brothel management game with many skills and activities that displays pictures or animations. Just delete the girls it comes with and install male character packs from the same website.

A few male characters are available in the main characters thread (I have two or three. Many dead links.) There's also some (male version of Loli? ah: Shota) male chars, in a subforum you need to register for.

The character stats are in easy-to-modify XML format, and it comes with a character editor so you could even create new characters from photos or screenshots.

School management games: I think Hentai High School lets you play gay male characters, so does School of Smut (text game). I've only tried these for a few ingame days each though, not even sure if HHS has any sex scenes.

Skyrim, Fallout 3, Fallout New Vegas, Fallout 4: There's many mods that work with a male protagonist, and a handful that focus on gay content. (I guess if you're into RPGs, you'll know about these here on Loverslab already.)

The Cathouse Tale seems quite early in development ... you walk around a brothel as a brightly-colored humanoid (cat if I remember right) with a huge erection and talk to other hybrid creatures. Not sure if there are any sex scenes to trigger yet. The team does seem to have big plans though, the demo's stable, and they say "We intend to make sexual content for everyone to enjoy, featuring every combination of males, females, and ermaphrodites, so you'll see plenty of variety, but with the option to restrict scenes according to your personal tastes." so maybe check them out.

Lust Doll - almost an art project / experimental game. It uses the RPGMaker engine, but feels notably different from other RPGMaker games.
