Character model disappear instead of fading out when up close to the camera in third person

Just like the title says..
when this happens:
 

some time after entering soul cairn (Dawnguard DLC) when stepping on a teleporter (the one used to reach a Reaper gem fragment), this happens even after I complete all quests in there and back to Volkihar castle

 

I've tried to load another healthy save and load this save, but it's still there.

 

EDIT: it looks like i have a safe save in soul cairn when my character still fade out and transparent instead of disappear and complete invisible when close up to camera. The cause is teleporter visual effect (based on the test run with safe save). 

 

anyone got any issue like this?
