I admit I've never noticed this on the mtleft, probably it's hard to notice it while you play in third person because of the body covering the arms. Then after a while I started not using it (the mtleft) anymore. Can't say if noone has ever solved it, it would deserve checking some run replacer, for example I've seen a new run on nexus like 1 month ago.

They were introduced in the BnB extended skeleton (or what was the name of that skeleton), along with many other bones, I personally have never seen any of them used in any anymation. I think I've never seen an upperbody rigged to that skeleton, too. There are far too many things that never be done, in terms of animations, unfortunately there are not many animators around.
