Player Character being launch at random.

For some reason, my character has started teleporting or just getting launched (No ragdoll, just moved as in "sliding") at random times when getting hit. It doesn't matter where the hit comes from, or from what, but it will teleport the player a short distance from time to time. 

 

No apparent effects are on, and the issue just started happening all of a sudden (No changes were made) This has been happening for a while now and it becomes unplayable when the character gets launched miles away from combat after a few hits. (Health bar has nothing to do with the issue, nor the damage/force of the attack)
