People randomly becoming yellow triangles

for some reason people are randomly becoming yellow triangles, and it appears the heads are missing. Have No Idea why this is happening.

Decided to load a previous save and that seemed to fix it, but this is the second time something like this happened to me, so finding out what the heck is causing this would be great
