Strange bugs - armor removed when killed, followers changing armor randomly

These aren't particularly gamebreaking in any way, but I'd like to make them stop regardless. I have a feeling they may be related.

 

The first bug only occurs when my character is killed. She ragdalls to the ground as normal, but then all of her armor is suddenly unequipped with the exception of her boots and underwear accessory (as if a sexlab animation was starting up? Ick). She starts twitching on the ground until the game automatically loads my last save. 

 

The second involves my follower (Cerwiden, if that's important, but others have done this as well) randomly unequipping all of her armor and then immediately reequipping them a few seconds later. This usually happens when loading a save or entering a new area. My own character has done this occasionally as well.
