Ostim is modernized and has better visual quality, however few quest mods support.

 

If you're looking for a sex scene generator, click NPCs to start or initiate a scene with one or two dialogues, then Ostim is no doubt better

If you're looking for lore and immersion, a complicated quest with spicy content, espesially those with non-consensual and beast content, sexlab is essential

All sex system frameworks are build on sexlab. I see a lot of ostim patch these days though, sexlab is still a common requirement for loverslab mods
