Can I know what FSMP version are you using? I tried reinstalling with the latest V4.1.1 and they are still bald

From what I know 4.1.1 is rather new and use SKSE Menu Framework, if you still use MCM get version 3.5.0

OK good news after extensive retesting my load order in every way, it turns out the real culprit was the mod - "Racial Skin Variance"

 

So to solve this, we just need to create a new RSVIgnore INI Distribution file and include all the editorID of the NPCs

 

Well in case anyone else is also being affected by the baldness (or just using RSV in general) - you can try the INI I've attached, unzip directly on the mod folder where your ESP is located at

In my list it just leads to the game loading endlessly, hanging before the menu even comes up.

Can you open your SSEdit and take a look at Lexanne Capulet AI? Does it show empty or show a match like this?

It looks different: (BoS 1.9.0.7) I've updated to the latest version now and that fixed it. (1.9.1.4)
