You likely have a mod installed that is causing this... one that purports to change how NPCs dress and that does not account for users who want their NPCs naked.

 

There is nothing within the vanilla game or SL itself that does that.

Do you have CBBE 3BA installed? iirc, one of the default MCM hotkeys for that mod shares a slot with either a Sexlab or OSex/OStim hotkey that you might be pressing that causes that.

I think I've figured it out, it must have been the dw's CBBE 3BA with UBE Anus mod causing the issue. No problems since I switched to Touched by Dibella. A shame there is no anus on that one.
