Only time I see this bug is when an NPC goes through a door, it takes like 5 seconds before the door can be used again by the player. In this situation it remains ajar but it never triggers the cell change.

That's because its the door that does the cell change, not the black hole.

Could also be some mod broke the link between the load doors:

  1. Go to the broken door.
  2. Open the console.
  3. Click on it, get its reference id.
  4. Open xEdit.
  5. Find the REFR record. Pay close attention to the XTEL field, and any changes made to it.
  6. Also inspect the teleport destination door in the REFR record, make sure it exists, and hasn't been modified.

If it still doesn't work, inspect the doors in the CK: Make sure they both have teleport destination markers, and that these are placed on navmeshed ground.
