Crash when I get into combat

A little after I enter combat and attack an enemy my game CTDs and i cant figure out why. If anyone can offer assistance it would be appreciated!

Your issue is coming from audio.

1. Disable or otherwise change all audio mods you may have.
2. Make sure all audio files have the correct number of channels (mono usually), the correct frequency and bitrate.
3. Make sure your OS audio settings are correct. Don't push 7.1 audio to 2 channel headphones, don't set the frequency too high: 44khz is perfectly fine.
4. Try removing any weird audio devices you may have, such as headphones that show up as USB sound devices.
