Character changing genders upon loading

Whenever I load into the game, theres a 90% chance that my female character will have turned into default Nate (with blonde hair, cause that's what I had)

 

This affects all saves from my character, but it doesn't happen to my sister.

Any ideas what might be the problem?

How long has this been happening? Is there anything you've done around the same time that could have caused it? Which version of the game are you using - the "next-gen" version, or the one from 2019?
