I can't get cas to behave properly

I want a trap/femboy sim 

Problem is the game doesn't want to work with me and gives me ether an obesed male sim or a anorexic male sim 

Want a femboy that basically looks like a woman but with a flat chest and a penis something like this

I did that and while slim it gives the sim the anorexic affect

It could be the fault of a slider, do you have sims 4 studio (s4s)? With that program you can update sliders.
Also, have you already tried to adjust it with these tools?
