Looking for mod [User Interface (UI)]/Graphics

Im kinda dumb (& intoxicated at the moment, so plz forgive), I am looking for a mod that gives visualisations.

Something like [attached], I have no idea where this img is from & i do not know what enables such imagery/graphics

 

 

Any assistance would be greatly apprechiated, & please ensure your awnsers are helpfull in someway. Also keep your explenations simple as I am:
