I installed a new penis skin and it only changes in the create menu but within the game it still has the default evil Wims mod, I want to change the default penis within the game

Are you talking about when your sims Shower? Because that "Nude," outfit doesn't seem to change from the default WW one.

 

In terms of animations though, it should use the default replacement you picked when animations trigger.

 

You can try going into the settings in game by clicking on a sim - WickedWhims - Anatomy - And then select Soft/Hard penis and choose "All sims," and pick the one you want. It should change all the currently made in-game sims for you
