Inconsistent Performance and severe lag

As the title says. I have been tinkering on my mod build for the last few weeks but no matter what I do, I get absolutely dogshit performance when I'm outside - weirdly enough, not all the times, sometimes its very stable, other times I'm down to 4 FPS and I have 0 idea what causes this. Maybe anyone here has an idea.
