Sim gets pregnant without doing anything with no baby after 'giving birth'

So like the title says, everything was going smoothly until my female sim got pregnant for the very first time...after that, without having any sex, just by going jogging or having a bath, or even just waiting, she appears to be pregnant again but of course at the end of the pregnancy there is no baby...
I havent added anything other than animations and im using the latest WW and as far as i know my few mods like nisa, etc are all up to date

i have similar issue in 174a, though i didn't wait till her pregancy end.

i used mccc to track pregnancy, mccc told me she is not pregnant, but the bloated stomach and various bubbles behave exactly like a pregnant women.

i used mccc to start pregnancy on her, then end.

 

didn't have further issue with her.

 

but now i similar issue on another npc sim (not playing that) in 174b, mccc told me she is not pregnant but the visual told otherwise.

haven't done anything over this yet.
