bodytalk options only available at game start char creation?

i dont know wat i am doing wrong but during the playthrough i found my npc's and male toon just was a bit to grose and i cant seem to be able to activate the added options from bodytalk when visiting barber / surgion or even using the console to adjust my face?

does any one have a possible sollution i know it works or u could not adjust some at char develeopment  just some how it wont initiate at any other moment  so far?
