It looks like it's female skin.

Skins are either male or female, and the female version's chest is different. Sometimes skins have a male and female version, a merged version (both in one file), or only a version for one of the two.

 

I downloaded some nice skins but they were female only so they created a similar effect on my sims and I had to either) 1. Find the male version of the skin or) 2. Not use the skin on the opposite gender.

I didn't use any skins. Female sims also have multiple nipples. The npc sims are the same.
