Wearing restrictions not working (combat strip, etc)

I’m getting the damage and the items (ie !damaged right leg) but the armor is not affected and I can simply equip at will. 
 

also, collars meant to punish for wearing clothes…don’t

Combat strip needs to be configured if you want specific slots to be stripped / destroyed.  Check the MCM and use this guide to translate the slot that shows in xEdit (or if you have the rad morphing redux mod, there's an option in the mcm to show what clothing your slots are using) https://www.creationkit.com/fallout4/index.php?title=Biped_Slots

You want to configure the index number in Combat Strip Lite, so if you are wearing like normal vanilla outfit, it's going to be slot 33, which is Index 3

 

As for the second question, what mod is the collar from?  Devious Cursed Wasteland?

You may be right about it doing it by default, but when I was trying to figure out how the mod worked I just started configuring it manually after reading the description again.   So if it doesn't seem to be working the way you think, try configuring the advanced section of MCM.  The support thread for the mod may have additional information (The Get Support button on the download page).  I remember being able to re-equip things I thought were damaged but I believe they normally unequip themselves again until the "stacks" are healed.

 

As for the DCW collar, I've never used that mod but there are people that keep an eye on its support thread, so you could try asking there or check the posts to see if the question has been answered already.
