Broken body slide body.

Any idea what is causing this or how to fix it?

Looks like this has been asked before.  Read the section by AWP3RATOR.

 

You should also include mod load order and not just the plugins.  You may have a texture/mesh issue which could be in a mod that does not use a plugin.

SOLVED: the issue was enhanced vanilla bodies. uninstalling that fixed it.
