Very odd 0% vats hit chance on limbs without armor

Playing heavily modded non-adult (mostly) fallout 4 setup and I have run into an issue with any female (and only female) targets having 0% hit chance in VATS, but only on limbs (arms, legs) and only if they're wearing no armor piece on that slot. 

 

Naked females, or females wearing only an underarmor or clothing outfit would have a hittable head and torso, but arms and legs are 0%.  If that same character were to wear leg armor (let's say light leather), the legs would now both have a nonzero percent to hit in VATS.

 

Interestingly, getting the Penetrator perk gives a regular hit chance in any condition.  So it's like the meshes for the arms and legs are hidden behind something the game is treating as an obstacle rather than clothing.

 

Removing the mod "Female outfit Replacer" by lazman seems to remove this issue, however I've had old versions of this mod in the past that didn't do this, so I think it's some kind of interaction.  I've even loaded old version of his mod (last one I had from my last install, maybe 2 or 3 years old) and an old version of CBBE from that era and replaced the current versions and the issue still persists.

 

If someone is interested to help out, i'll post a load order but if someone knowledgable can tell me what kinds of mods could even produce this odd behavior I can probably track it down.

Step two, I uninstalled ALL my mods.  Installed (2nd time) NEW CBBE (reduced), bodyslide, Female Outfit Replacer - deleted all my meshes everywhere just to be sure, re-ran bodyslide, generated new meshes.

 

Create new game (in case some script got broken), and now arms, torso, head on females wearing no armor, or only clothing, can be hit in VATS.  Legs cannot (0%).  Unclothed, unarmored npcs can be hit once more.

 

Same if I repeat the whole process with other clothing overhauls (2pac's for example).

 

Is bodyslide doing something funky?  Very confused.
