dark vanilla underwear bug help!

seen many guides but they haven't answered well, at least to me they haven't. recently my SOS has been leaving the vanilla underwear on, on some models but its dark underwear and the penis goes through it. and sometimes theres a texture going around from butt and crotch on some characters anyone know what could be the problem?
