sims showering/bathing in their underwear

Has anyone come across a situation where your sims gets into the shower or tub still dressed. mine have started doing that and I can't figure out what's causing it.

Any suggestion?

further to my last it seems to happen (showering with their clothes on) when they switch to going nude.

it doesn't happen to everyone.

seems I have something in my files that assigns clothing / accessories to randomly to the nude default.

I've tried (cas.fulleditmode on) cheat and the sim comes up with their properly assigned clothes.

is there a cheat that brings the sim up in CAS naked so i can remove the offending articles?
