Unfortunately you can't size the head up in Black Desert. Meaning if you create something 7B like with broad shoulders, you best go for a broad haircut, cause otherwise it will look like shrunken heads.

No this is a real deal, using ResOp which is a DX11 hook texture replacer. 

The armor is actually standard, but using ResOp and an xml patch to replace base game underwear and starting outfits means that underwear is permanently disabled, it just so happens that this specific outfit is built with a mesh based alpha channel and if you simply dye it with the in game dye system with a non-bright diffuse color, the alpha channel becomes apparent and putting it all together you get the above.

That means that they actually build nipple & areola into the game. Wonder why that happened.

I'm replacing the body textures with ResOp as well, the model does have a vestigial nipple designed into the mesh though and the base game normals have it as well. 

Ohh so this isn't skyrim .
