OSex actor "teleporting" back after Scene Start

I'm having a very frustrating issue with the sub actor "teleporting" that I can't seem to fix:

I've tried a complete uninstall/reinstall of Skyrim and my mods to try and fix it, with no luck.  The most frustrating part is that OSex has been working fine for months, then this suddenly started happening.

 

Any clues where I should be looking to troubleshoot? I've tried:

1. Complete reinstall of everything Skyrim/mods
2. Disabled all mods except OSex, Netimmerse override, Racemenu
3. Redownload/reinstall OSex

I've been using version 1.09, although I did try the latest Nexus version just to see but it had the same issue.
