CTD via Death load... I don't understand anymore.

Alright, so I kind of have been asking this everywhere but here, however after redoing my load order, I figured I ask the one place that helped me big last time.

alright, so basically my issue currently is that I CTD after death inside swindlers den with a random chance of the game loading without CTDing. in this case, the save is on an exterior cell while the death is in an interior. So far I have gotten very frustrated by it, but I can't seem to diagnose the issue despite loading a new game and slowly adding mods then trying to see what kicked the bucket. I also tried to do it in reverse; use the original save and slowly Remove mods. That method obviously didnt work, so I redid the load order, hoping to fix it. however, I somehow stumbled upon almost identical situation as before; level 6, with follower, yennefer armor on, so I decided to test it one more time. And of course, the first death succeeded to load the save, with the next failing. The starting point of the test is always the lake just east of Swindler's Den.

I should note that even though I have OSA poser module installed, none of the poses are actually in currently. I did this to make sure that it wasn't a Footlk issue like I had a while back.
