If you mean just the horse and if it is Arvak then it comes from a side quest in the Soul Cairn, part of the Dawnguard questline. 

 

If you mean with the armour as shown in the image, sorry cant help you there but I would guess (not tried it) that convinient horses could apply one of its armours to Arvak, just not the one in the image as it isn't one of theirs.

I agree, but it looks like the hair is "bending inwards"(?). Maybe there's a patch for that hairmod for use with headwear.

the face seems to be purely made through ECE with the hair being from SGhairs from what i cant read

 

also the armors might be in the Aether Suite

which seems to have been taken down from nexus
